Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: a)At the facility, on or about 12/30/05 the following elevator deficiencies were noted: 1)Maintenance elevator used to move parts and equipment between three floors was not provided with a rear gate and was not provided with an interlock system to prevent the car from moving with the gate open. 2)Production elevator used to move finished belts between floors was not provided with an interlock system to prevent the car from operating with the gate and hoistway doors in the open position. 3)Maintenance and Production elevators have not been inspected in accordance with the requirements of ANSI/ASME A17.3a-1986, Safety Code for Existing Elevators and Escalators, Section 1.5. Among other methods, acceptable and feasible abatement methods to correct these hazards are to: 1.Perform routine and periodic inspections of the elevators in accordance with ANSI/ASME A17.3a-1986, Safety Code for Existing Elevators and Escalators, Section 1.5 and ANSI/ASME A17.1. 2.Provide an interlock system, i.e. an electric contact that shall be positively opened by a lever or other device attached to and operated by the door or gate, and that shall be maintained in the open position by the action of gravity or by a restrained compression spring, or both, or by a positive mechanical means in accordance with ANSI/ASME A17.3a-1986, Safety Code for Existing Elevators and Escalators, Section 3.4.2, Car Enclosures. 3.Fully enclose all nonentrance sides and top of elevator cars in accordance with ANSI/ASME A17.3a-1986, Section 3.4.1 for passenger and freight elevators. b)At the facility, on or about 1/5/06: The following hoists were not being inspected on a frequent and periodic basis:1)Jet 1/2-ton Hoist, near oven; 2)Jet 1/2-ton Hoist, above segment table; 3)Jet 1/2-ton Hoist, wide table area. Among other methods, one feasible method of abatement is to conduct frequent and periodic inspections of the hoists for wear, deterioration and malfunction as per ASME B30.16-1993 c)At the facility, on or about 1/5/06: The support structures and under the hook lifting devices for the following hoists had unknown load capacities: 1)Jet 1/2-ton Hoist, near oven; 2)Jet 1/2-ton Hoist, above segment table; 3)Jet 1/2-ton Hoist, wide table area. Among other methods, one feasible method of abatement is to inspect the support structures for the hoists as per ASME B30.16-1993, to ensure the structure is designed to withstand the loads and forces imposed by the hoist for the rated load. Conduct frequent and periodic inspections of the below the hook lifting devices and mark the lifting device with rated load capacity as per ASME B30.20-1993. d)Near the Oven, on or about 1/5/06: Jet 1/2-ton hoist was being used at its maximum load capacity rating without factoring in the weight of the below the hood lifting device. Among other methods, one feasible method of abatement is to not load the hoist in excess of its rated load as required by ASME B30.16-1993, Section 16-3.2, Handling the Load.ing
