Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm in
that employees were
exposed to a struck by hazard:
a)Roof of building, on or about on or about 11/30/06:  Wooden roof trusses
were installed without proper continuous lateral and diagonal bracing.
Among other methods, one feasible and acceptable abatement method to
correct the hazard is to:  Install continuous lateral bracing and diagonal
bracing simultaneously with truss erection, as specified by Building
Component Safety information BCSI 1-03 - Good Practice for Handling,
Installing,  & Bracing Wood Trusses, 2004 BSCI Summary Sheets - B1,
B2, B3.
b)First Floor- center of building, on or about 11/30/06:  Employees were
exposed to the hazard of being struck by the an overloaded JLG aerial lift.
Among other methods, one feasible and acceptable method to correct this
hazard is to discontinue the overloading of the aerial lift as required by
the JLG Operation and Safety Manual Page 1-7 and to support the trusses
using an alternate support system.
Abatement certification must be submitted for this item.
