Title 40.1-51.1(a), Code of Virginia: Employer did not furnish employment, and a place of employment which were free of recognized hazards that were causing, or likely to cause death or serious physical harm to employees in that: Employee was exposed to fall hazards while standing on the outrigger platform, when the platform came out from underneath him, causing him to fall a total of approximately 30 feet, and causing serious injury. One feasible, and acceptable abatement method among others to correct this hazard is to comply with the manufacturers installation requirements, contained in the instruction manual provided by manufacturer, which is included with equipment which states: Install the plank ties so that they hold the outriggers and the planks together, and Nail or screw the plank ties to the planks so that they do not move.
