Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees
in that employees
were exposed to struck by hazards due the workover rig's hoisting
(traveling) block was not
guarded to pervent the hoisting block from striking the crown:
a)B.W. Barbour 16-6 Oil Well, Wilson Workover rig, Serial Number 10320:
The employer did not ensure that the workover rig's hoisting (traveling)
block
was guarded to prevent it from striking the crown.
Among other methods, one feasible and acceptable method to correct the
hazard is to
equip the workover rig(s) with an anti-two block device, a Crown-A-Matic
device, or
other positive devices that will prevent the hoisting block from making
contact with the
crown block in accordance with API RP 54-99, Section 9, Sub-Section 9.7,
Paragraph
9.7.4. "Traveling blocks shall be properly guarded and shall not be
operated unless
guards are in place".
