On or about 02/02/06, at the forklift yard, employees were exposed to the
hazard of falling and
being crushed by powered industrial trucks while operating forklifts
without wearing restraining
devices such as, but not limited to seatbelts.
Among other methods of abatement, one feasible and acceptable method to
correct this hazard
is to prohibit employees from operating this type of equipment without
wearing seatbelts.
PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS
REQUIRED FOR THIS VIOLATION.
