Section 5(A)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and run over hazards: On or about 04/10/06, at the west field pad, employees were exposed to the hazards of being struck and run over by a forklift that was lacking an audible reverse signal alarm. Among other methods, one feasible and acceptable method to correct this hazard is to eliminate the practice of using defective equipment such as, but not limited to forklifts. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
