Title 40.1-51.1(a), Code of Virginia: Employer did not furnish employment, and a place of employment which were free of recognized hazards that were causing, or likely to cause death or serous physical harm to employees in that: Employees were exposed to struck by hazards, when while removing cap from a six inch pressurized water line. The cap blew off and struck both employees. Supervisor failed to notify employees that the line was under pressure, and instructed these employees who were not familiar with, nor had they been trained to bleed air out of pressurized pipes to remove pipe cap. One feasible and acceptable abatement method among others to correct this hazard is to : Follow the recognized industry standard of ensuring that pipe lines are not under pressure prior to removing cap or performing similar operations, and ensuring employees who are performing these types of operations are knowledgeable of the hazards, and trained to perform these task safely.
