employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that the
seat/safety bar was malfunctioning which allowed the Bobcat skid steer
loader to be operated
with the seat/safety bar in the raised position, exposing the employees to
a hazard of being
ejected from the loader during operation:
a.Quincy Castings, Inc. located in Quincy, Ohio:  On or about January 31,
2007, the
employer did not ensure the Bobcat skid steer loader located adjacent to
the mold
dumping department was equipped with a functioning safety bar that would
prohibit
movement of the machinery if the safety bar were in the up position.
Employees could
be exposed to the hazard of being ejected from the loader during operation
if the safety
bar was not lowered.
No abatement certification or documentation is required for this item.

2007, the
employer did not ensure the #4 transfer ladle used by employees to pour
molten metal
on the pouring line was labeled as to its capacity.  The ladle can carry
up to 854 pounds
of molten metal and have a total weight of 1600 pounds.
b.Quincy Castings, Inc. located in Quincy, Ohio:  On or about January 10,
2007, the
employer did not ensure the #1 transfer ladle used by employees to pour
molten metal
on the pouring line was labeled as to its capacity.  The ladle can carry
up to 854 pounds
of molten metal and have a total weight of 1600 pounds.
c.Quincy Castings, Inc. located in Quincy, Ohio:  On or about January 10,
2007,
the
employer did not ensure the "C" transfer ladle used by employees to pour
molten metal
on the pouring line was labeled as to its capacity.  The ladle can carry
up to 854 pounds
of molten metal and have a total weight of 1600 pounds.
d.Quincy Castings, Inc. located in Quincy, Ohio:  On or about January 10,
2007, the
employer did not ensure the spreader bar used by the weight shifter to
transfer 300 pound
weights from the tops of the molds in the pouring department was labeled
as to its
capacity.
Among other methods, one feasible and acceptable method to correct this
hazard is to
ensure the under the hook lifting devices are labeled with their capacity
according to
ASME B30.20a-2001 "Below the Hook Lifting Devices" and ASTM E2349-05
"Safety
Requirements in Metal Casting Operations: Sand Preparation, Molding, and
Core
Making; Melting and Pouring; and Cleaning and Finishing"
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this
violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

employer did not ensure the monorail hoist used to lift the #1 molten
metal pouring ladle
on the pouring line was equipped with a hook safety latch.  The ladle can
carry up to 854
pounds of molten metal and have a total weight of 1600 pounds.
c.Quincy Castings, Inc. located in Quincy, Ohio:  On or about January 10,
2007, the
employer did not ensure the monorail hoist used to lift the "C" molten
metal pouring
ladle on the pouring line was equipped with a hook safety latch.  The
ladle can carry up
to 854 pounds of molten metal and have a total weight of 1600 pounds.
Among other methods, one feasible and acceptable method to correct this
hazard is to
ensure the hoist hooks are equipped with safety latches according to ASME
B30.16-2003
"Overhead Hoists (Underhung)".
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this

unintended direction during
furnace pouring operations:
a.Quincy Castings, Inc. located in Quincy, Ohio:  On or about January 10,
2007, the
employer did not ensure the pendant control buttons were properly labeled
for the 5 ton
single girder underhung crane used to hold the transfer ladle for the 32
ton Ajax furnace
in the melt department.
Among other methods, one feasible and acceptable method to correct this
hazard is to
ensure the pendant control buttons are properly labeled as to their
function according to
ASME B30.11-1998 "Monorails and Underhung Cranes".
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this
violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
