Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees, in that: the
operators working with the 10-ton Erie Products top running bridge crane
with a Shawbox
underhung hoist in the melt area of the foundry were exposed to the
potential of being struck
by materials carried by the crane if the material were to be disengaged
from the hoist due to the
hook not having a safety latch:
a) B.W. Grinding, Inc. - Lyons, Ohio - On or about February 8-9, 2007, the
employer did not
ensure that the hook for the 480V electric Shawbox wire rope hoist in the
melt area of the
foundry was equipped with a safety latch.
Among other methods, one feasible and acceptable method to correct this
hazard is to ensure that
the hook safety latches are installed, per ANSI B30.16-2003, (Overhead
hoists,
Underhung).
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
written records.

operators working with the electrically powered 480V Erie Product top
running crane with an
underhung 10-ton Shawbox hoist were exposed to the potential hazard of
being struck-by
materials carried by the hoist due to improper handling of the load and
leaving the controls
unattended while the load was suspended:
a) B.W. Grinding, Inc. - Lyons, Ohio - On or about February 8-9, 2007, the
employer failed
to ensure that the operator of the underhung 10-ton Shawbox hoist in the
melt area of the
foundry did not work under a suspended load, in that, the operator was
standing directly under
an approximate 300-400lbs. mold to apply burn-off product to the bottom of
mold.
b) B.W. Grinding, Inc. - Lyons, Ohio - On or about February 8-9, 2007, the
employer failed
to ensure that the operator of the underhung 10-ton Showbox hoist in the
melt area did not leave
his/her position at the controls, in that the controls were unattended
when the operator was
standing under the approximate 300-400lbs. mold to apply burn-off product
to the bottom of
mold.
Among other means, one feasible and accepted method to correct these
hazards is to ensure that
the operator shall not carry loads over people and shall not leave the
position at the controls
while the load is suspened over an area accessible to people, per ANSI
B30.17-1998, (Overhead
and Gantry Cranes - Top running bridge, single girder, underhung hoist).
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
