Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to crushing injuries in the event of a loss of load
while operating an
underhung crane that is not inspected on a regular basis:
(a)Shop Area.   The Shawbox underhung 10 ton crane, S/N 58580, did not
receive
frequent and periodic inspections. The most recent inspection was
conducted on May
3, 2005, as observed on September 27, 2007.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to have written, dated, and signed inspection reports and records made
on critical items
such as hooks, brakes, ropes, chains, trolley swivels, and safety devices,
as specified by
section 11-2.1.5 of the American National Standard ANSI B30.11-1973,
"Monorail Systems
and Underhung Cranes".
