Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free form recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and fall hazards in that: a.Employees of Trailer Specialist, LLC work on an elevated work platform attached to a Daewoo powered industrial truck, that did not have the following information indicated on the platform: maximum load including personnel and equipment and weight of empty platform, as determined on 01-12-06. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to use a work platform that is specifically designed for use on fork lifts and meets the requirements of ASME B56, 1-2000, Safety Standard for Low Lift and High Lift Trucks, paragraph 7.36.3(i). b.Employees of Trailer Specialist, LLC work on an elevated work platform, 5 to 10 feet above the ground, attached to a Daewoo powered industrial truck. The guardrail system on the platform did was not provided with midrails, as determined on 01-12-06. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to use a work platform that is specifically designed for use on fork lifts and meets the requirements of ASME B56, 1-2000, Safety Standard for Low Lift and High Lift Trucks, paragraph 7.36.1(d).
