Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Employees working with, and in the vicinity of, loads suspended from the 1
ton jib crane in the
Blohm Grinder Room were exposed to the hazard of being struck by falling
loads as a result of
improper rigging in that eye bolts used as lifting eyes were the incorrect
type or size bolts for
the purpose. In some instances the eye bolts were too small to fully
accommodate the load hooks
of the double legged chain sling that was used to attach the load to the
crane. In other instances
shoulderless eye bolts were used to rig the loads and were then pulled at
an angle, or shouldered
eye bolts were either not inserted to their full depth or were not used
with spacers to shim
between the bolt's shoulder and the load surface.
Among others, feasible and acceptable means to correct this hazard would
be to follow safe
rigging practices for the use of eye bolts such as, but not limited to,
those published in The
Rigging Manual of the Construction Safety Association of Ontario.
