Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) Saw Department, South Wall -  The Parma Workamatic vertical band saw,
used to cut plastic
parts was not equipped with a magnetic restart or similar device.  The saw
was used last on or
about November 22, 2005.
(b) Saw Department, North End - The Delta Shopmaster vertical band saw,
used to cut plastic
parts was not equipped with a magnetic restart or similar device.  The saw
was used last on or
about November 14, 2005.
Among other methods, one feasible and acceptable method to correct his
hazard includes but not
limited to the following: NFPA a National Fire Protection Association
electrical
standard for
Industrial Machinery NFPA 79 - 2002, Section 7.5.3.  Upon restoration of
the voltage or upon
switching on the incoming supply, automatic or unintentional restarting of
the machine shall be
prevented when such a restart can cause a hazardous condition.
In accordance with 29 CFR 1903.19, abatement certification is required for
this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation include, but not be limited to,
evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
written records.
