Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards:
(a) Location - 200 Wheeler Road, Burlington, MA:
On or about 12/01/05, an employee was exposed to a fall hazard of up to 40
feet while
using a 5/8" Kernmanatle rope that was in poor condition and failed.
Among other means of feasible means of abatement would be: 1) Train
employees in the
ANSI/IWCA I14.1 2001 Standard, related OSHA safety standards and the
manufacturers
operating/safety instructions. 2) Ensure employees conduct a daily
inspection of
equipment to be used daily. 3) Ensure when using a roof rolling suspension
unit it has
the required amount of counterweights for a 4 to 1 safety factor. 4)
Ensure
points for tiebacks and lifelines are capable of supporting a 5000lb load
and independent
of each other. 5) Ensure employees lifelines and tieback lines are not
more than a 15
degree angle. 6) Ensure a written workplan is inplace onsite and reviewed
with
employees prior to any work.
