of a forklift that was not properly used and/or secured:

On or about 1/27/06, employees were riding the forks in a cage that was
not properly
installed and/or secured.
Among other methods, one feasible and acceptable means of abatement is to
follow the
recommendations of American Society of Mechanical Engineers (ASME)
B56.1-1993,
which includes but is not limited to, means to securely attach the
platform to the lifting
carriage or forks, and to prevent the lifting carriage or forks from
pivoting upward, and
a platform width not greater than the overall width of the truck plus 10
inches on either
side.r
