Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees due
to use of a defective
safety latch on the hook of the crawler crane:
On or about May 25, 2006, at the Pioneer Rig #32 work site, a Kobelco
CK850-II
crawler crane was used with a defective safety latch on its hook.
Employees were
exposed to the potential of being struck by a load in the event that its
sling may
accidentally roll or slip off the hook.
Among other methods,one feasible and acceptable abatement method is to
provide and maintain
safety latches on all crane hooks in accordance with American National
Standard Institute
(ANSI) B30.10-1975, American Society of Mechanical Engineers (ASME)
B30.2-2001,
and as
required by OHSA standard 29 CFR 1910.181(j)(2)(ii).
