Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees:
On or about September 19, 2006,and times prior thereto, no emergency escape
descent device was installed on the derrick of Rig #6, located at a site
off of CR
3600, East of Longview Tx. Employees were exposed to the hazard of fire,
explosions, and no escape route from the pipe racking board.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
would be installation of a properly installed and secured emergency
descent or other
satisfactory auxiliary means of egress, such as specified in the American
Petroleum Institute
