recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall and crushing hazards:
On or about March 18, 2007, at a workplace located at 6602 Dalrock Rd.,
Rowlett, TX, at least
two employees, who were riding and working from the loader bucket of a
Caterpillar, 416E,
backhoe loader (serial number G4D10114), were exposed to the hazards of
falling from the
bucket, and being crushed-by the tires of the moving machine.
Among other methods, at least two feasible means of abatement include but
are not limited to:
(1) Comply with Caterpillar's safety instructions contained in the
operation and maintenance
manual which state, "Do not allow riders on the machine unless additional
seat, seat belt, and
Rollover Protective Structure (ROPS) are provided; and (2) Comply with
Data
Sheet 1-589 Rev.
90, published by the National Safety Council which states, "Operating
Safety.  Do not allow
riders other than the operator on the machine.  Do not ever use a loader
as a work platform or
staging..."
