Section 5(a)(1) of the Occupational Safety & Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to falls and caught-between hazards from the failure of an airplane boarding bridge where the company did not conduct periodic preventive maintenance of the bridges. The racking limits had been defeated in that the switch housing mounting bolts had been loosened. This condition would have been identified during scheduled preventive maintenance inspections, which had not been conducted: Jobsite: Terminal B, Gate 6. Employees and passengers were exposed to falls and caught-between hazards where the FMC Jetway boarding bridge malfunctioned and collapsed. One method of abatement, among others, is to implement a preventive maintenance program where manufacturer's suggested scheduled inspections would be conducted by authorized and competent mechanics.
