Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falls from ladders and ladder collapse:
(a)On or about 02/15/2006 - Sanitation - Employees performing sanitation
operations
were exposed to falling from 6' and 8' portable stepladders while standing
on the
top step of the ladders and reaching out.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to follow and implement the manufacturer's requirements which states as
marked on the ladder,
"The top or the top step of a step ladder shall not be used."
