Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)Front of building at 141 Kelly Dr, Dothan, AL: On August 24, 2006 an
employee
was exposed to a fall of 12 feet to ground while working from a metal
personnel
platform installing metal studs and the platform extended 45 inches beyond
the
wheelbase on each side of the Lift-It Model 50226 (S/N 39096870) rough
terrain
forklift.
Among other methods a feasible and acceptable means of abatement of these
hazards are to
follow the requirements of ANSI B56.6-2000 for the use and design of
personnel platforms.
Paragraph 8.24.1 (b) states: Floor dimensions shall not exceed two times
the
load center distance
listed on the rough terrain forklift nameplate, measured parallel to the
longitudinal center plane
of the truck, nor have a width greater than the overall width of the truck
measured across the
load bearing tires plus 10 inches (250mm) on either side.r
