5(a)(1):  Section 5(a)(1) of the Occupational Safety and Health Act of
1970:  the employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing or likely
to cause death or serious physical harm to employees.
Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  On
February 1, 2006 the employer did not
furnish employment and a place of employment which were free from
recognized hazards that were causing or
likely to cause death or serious physical harm to employees, in that
employees were exposed to crushing and
impact hazards caused by the unexpected ejection of a 2,705 lb. work part
in the process of being machined,
from the Schiess Single Column Vertical Boring Mill, S/N 1171S025, located
in Plant 3.  The following
conditions contributed to the hazardous condition: a) the use of aluminum
shims between the machined part
and the chuck jaws;  b) no guards and/or protective shields of sufficient
strength to protect employees from
potentially ejected parts in use; c) no established inspection,
maintenance, and repair program for the
mechanical and electrical vertical boring mill components and chuck
mounting
hardware; to identify and
correct problems such as, but not limited to; failure/malfunction of the
control system causing unexpected
table overrun/overspeed; failure of the workpiece clamping hardware
resulting in loss of stability and ejection
of the parts. Feasible and acceptable abatement methods to correct this
hazard may include: a) the proper use
of chuck soft jaws in lieu of aluminum shims when clamping the part in the
fixture; b) conduct a Task
Hazard/Risk Assessment (As identified in ANSI B11.23-2002 Safety
Requirements for Machining Centers and
Automatic, Numerically Controlled Milling, Drilling and Boring Machines,
clause 5 Hazard Control, sub-
clauses 5.1 and 5.2) prior to the start of each new job to ensure adequate
employee protection from potentially
ejected parts by shields of sufficient strength (including means of fixing
to the machine/floor) to contain these
parts. (As identified in sub-clause 6.23, of ANSI B11.23-2002.); and c)
follow the supplier's/manufacturers
instructions and recommendations (found in the Schiess AG Machine Manual)
to establish procedures for the
inspection, maintenance and repair of the vertical boring mill mechanical
and electrical components.  (As
identified in sub-clause 9.4 Maintenance, of ANSI B11.23-2002.)

before March 02, 2006 the
employer did not furnish employment and a place of employment which were
free from recognized hazards
that were causing or likely to cause death or serious physical harm to
employees, in that employees were
exposed to crushing and impact hazards caused by the unexpected ejection
of a work part in the process of
being machined, from the Dainichi horizontal CNC turning machine, company
No. WC289, located in Bay 3
of Plant 1.  The turning machine would run in automatic mode of operation
without monitoring the
workholding device actuating force, allowing the turning machine to run
without a part properly clamped
and/or gripped.  One feasible  and acceptable abatement method to correct
this hazard would be to comply
with ANSI B11.22-2002 Safety Requirements for Turning Centers and
Automatic, Numerically Controlled
Turning Machines, sub-clause 6.17.2 Power operated workholding device.
Ensure a program will not run in
the automatic mode of operation unless the workholding device actuating
force is monitored (e.g., by
monitoring the hydraulic pressure of power operated workholding devices).
If the required actuating force is
not reached or the necessary remaining stroke is insufficient it shall not
be possible to start the workholding
spindle drive.e
