On or about March 6, 2006
a)  An employee was exposed to a fall hazard of approximately 11
foot while emptying a 500 gallon drum from a broken lifting
platform that was elevated by a fork truck. The platform does not
meet the design, construction and use requirements in accordance
with ASME B56.1, American Society of Mechanical Engineers
Safety Standard.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to: a) provide a safe work platform in accordance with ASME B56.1-2004,
American
Society
of Mechanical Engineers Safety Standard for Low Lift and High Lift Trucks.
NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION
FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.CE
