North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a)Facility - - Dyeing Department: First level hoistway gate for the material hoist to the dye mixing area was not interlocked. NOTE:Among other methods, one feasible and acceptable method to correct this hazard is to install an electrical interlock on the first level gate at the hoistway for the material hoist such that the gate cannot be opened while the hoist is in operation or the material platform is not at the lower level.
