and a place of employment which were free from recognized hazards that
were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
struck-by and crushing hazards:
Employees performing daily operations while working in material
shipping/receiving, staging and CNC areas
were exposed to hazards associated with struck-by and crushing. Plate
hooks were made in-house and used for
lifting of steel plate ranging in size of 8 feet wide x 20 feet long
(approximately 3200 pounds) to 8 feet wide x
40 feet long (approximately 6400 pounds) without being load rated.

AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS
HAZARD IS:
a) Comply with ASME/ANSI B30.20 "Below the Hook Lifting Devices" and
ASME/ANSI B30.10 "Hooks"; 1)
Provide rated load on any custom-made lifting device. 2) Provide initial,
frequent and periodic inspection of the
lifting devices. 3) Ensure that initial proof testing of 125of the rated
load was conducted.
b) Implement a thorough schedule preventative maintenance program as well
as pre-lift inspections and tests as
prescribed by the ANSI/ALI ALOIM-2000.
Abatement Note: Abatement certification is required for this item using
the CERTIFICATION OF
CORRECTIVE ACTION WORKSHEET.
