from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in
that employees were exposed to:
a)site - on or about 03/03/06, employee was observed operating an
excavator, John Deere, model
27CZTS, product # ??027CX221825, when the installed and attached seat belt
was not fastened
and/or secured while performing excavation work.
One feasible and acceptable abatement method, among others, to correct
this hazard is to
develop, establish and/or update written procedures on proper equipment
usage,
train and/or
retrain employees on applicable and acceptable established procedures,
prohibit employees from
performing and/or attempting to perform inappropriate and unsafe
procedures, and enforce
established and/or applicable policies and procedures for equipment usage.
