Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by improperly secured equipment: New Rope Stretching Machine Area - Employees were exposed to the hazard of being struck by improperly secured equipment. The rope ladder (ladder tension system) pulled away from the base of the rope stretching machine causing the rope ladder to strike the leg of an employee who was adjusting the tension on the rope. The mild steel threads failed that were tapped into the mechanical box tubing of the rope ladder (ladder tension system). Stress on the two bolts that fastened the rope ladder to the machine pulled the mild steel threads out of the box tubing of the rope ladder. The machine was designed in-plant to pre-stretch rope and was in the development stage. Among the feasible controls for this hazard were: Fasten the rope ladder to the rope stretching machine with fasteners of adequate tensile strength. At the time of the inspection the hazard was abated by welding a bracket clamp to the machine and attaching the mechanical box tubing of the rope guide to the bracket with two Grade 8 1/2 - 13 through bolts each with double nuts.
