Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by falling rotary table:
(a)  On or about November 29,2006, ruffnecks on Rig#1-61 were exposed to
the hazards
of being struck-by a falling rotary table being pulled up the V-door
during rigging-up operations.
Among other methods, on feasible and acceptable abatement method to
correct this hazard is to:
1.  Instruct ruffnecks that standing on a load being moved, lifted, etc.,
by a forklift, air
hoist, cat-line(in this case the traveling blocks) is an unacceptable work
practice and should
never be done.
2.  Perform a job safety analysis (JSA) and instruct supervisors to use
the JSA as a
guidelines for completing a task.
3.  Establish procedures that ensure personnel and equipment are properly
employed
during the "rigging up" process.
4.  Ensure that safety is built into and practiced in all phases of all
operations and holding
those in the line of authority accountable for deviations from accepted
work practices.
