Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of being struck-by falling objects.
(a)  Wire rope clips on guy and safety line were put on backwards, located
on Rig#14.
One method among others would be to replace the clips with the type that
cannot be put on
backwards.  Ref:  29CFR1926.251(c)(5)(i) When used for eye splices, the
U-bolt shall be
applied so that the "U" section is in contact with the dead end of the
rope.
