Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of fall from elevation: (a) The wire rope clips to the emergency escape line were on backwards, located at the Golden Meadow location. One method among others is to replace the clips with fist grip dropped forged clips installed following the guidelines in 29CFR1926.251(c)(5)(i) When used for eye splices, the U-bolt shall be applied so that the "U" section is in contact with the dead end of the rope.
