Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of struck-by mast: (a) The wire rope clips on the guy wires of Rig #14 were of the wrong size and type. Two clips were not dropped forged and 3/4" clips were used on 1/2" wire rode at the Golden Meadow location. One method among others is to use fist grip dropped forged wire rope clips following the guideline found in 29CFR1926.251(c)(5): When U-bolt wire rope clips are used to form eyes. Table H-20 shall be used to determine the number and spacing of clips.g
