29 CFR Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did
not provide a work place which was free from recognized hazards that are
likely cause death or
serious harm to employees.  Employees were exposed to the hazards
associated
with an
unapproved work platform attached to the forks of a rough terrain forklift.
On or about October 18, 2006, at the above listed job site, an employee was
working out of an unapproved work platform that was attached to the forks
of a
rough terrain forklift, exposing the employee to the hazards associated
with the
platform collapsing.
A feasible means of abatement would be to follow the guidelines set forth
in the
American Society of Mechanical Engineers, ASME B56.6 - 2002, Safety
Standard
for Rough Terrain Forklift Trucks.
Abatement certification is required of this item in accordance with the
requirements of 29 CFR 1903.19(c)ts
