Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: (a) Location - 30 Bellevue Road, Arlington, MA: On or about 03/22/06, employees were exposed to a crushing hazard while using a crane with the Anti-two Block, and Load Moment Indicator safety devices inoperable. Among other methods, feasible and acceptable, to correct this hazard, include but not limited to: a) Follow manufacturers operators manual in safe use of the crane. b) Follow and utilize the ANSI/ISA Z133.1-2000 standard in crane inspection and safety measures prior to lifting employees by the crane.ees
