employees performing
roofing and decking operations were working from a forklift supported work
platform without
fall  protection and which far exceeded the platform width requirements by
62 inches, exposing
the employees to the fall hazards 30 feet from the platform or caught by
the SkyTrak JLG Model
8042 tipping over due to the size of the work platform.
A feasible means of abatement is to construct a work platform meeting the
requirements of
ASME B56.6-2002 8.24.-2002
