Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm: Rig # 5 well site: On or about July 10, 2006, the employer did not perform a rig inspection to ensure repair welds completely penetrated the structure to ensure sound condition; visible welds on the mast were rusting. Employees engaged in oil well drilling activities exposed to the recognized hazard of being struck by the mast in the event of structure failure. Feasible abatement: Ensure a written, dated and signed inspection report is generated to ensure critical items such as, but not limited to, the mast, draw works, snub lines, tongs, sub-structure and crown is addressed by the inspection process, per manufacturer's specifications. 0 0 y 0 L
