Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm:
Rig #62, Well site Kopf 1-34:  On or about July 27, 2006, employees
working on the derrick
engaged in fishing of an oil and gas well to wash sand out of the casing,
did not have immediate
access to the emergency escape device (geronimo) in the event of an
emergency.  The clevis that
held the emergency escape device to the derrick structure near the finger
board had broke and
the operator of the rig instructed that a piece of rope be used in place
of the clevis.  The rope,
with a knot tied in it, tying the cable to the mast failed to hold the
cable and emergency escape
device (geronimo) in place.  Among feasible and useful methods to correct
the hazard of the
broken clevis would include.
a)Using a substantial, temporary substitute to the original clevis that is
known to
be capable of withstanding 3000 pounds of force, as recommended by the
manufacturer;
b)Conduct on-site repairs using manufacturer's recommended practices to
reattach
the clevis, returning the part of the mast to the manufacturer's original
condition.er's
