Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: On or about February 8, 2007, employee(s) was exposed to the hazards posed by oil/gas well blowout, fire, and/or rig collapse. Employee(s) working at various level of a well service rig derrick was provided only one means of accessible egress, the derrick ladder, the use of which could be blocked by the preceding hazards. A secondary, emergency only, means of egress to remove the employee(s) from the proximity of the hazards would limit the exposure. A secondary means was provided but was not accessible, nor properly installed. Among other methods to provide a secondary means of egress, one feasible and acceptable means would be the installation and expeditious use of a properly installed and readily accessible "Geronimo" line. An employee was exposed to the noted hazards while working on the derrick of the employer's Rig 25 at the Irwin 1-9, Waynoka, OK.noka,
