SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that no seat belts for Skytrak 6036 forklift, used around job site, constituted a hazard of being thrown from the forklift. A feasible method, among others, to correct this hazard is to equip the forklift with seat belts and have operators wear them. B. Employer knew or should have known that no one at controls of Skytrak 6036 forklift, with hoisted personnel platform, in excess of 15 feet high, next to bell tower, constituted a hazard of being unable to quickly lower platform in case of an emergency. A feasible method, among others, to correct this hazard, is to have operator at controls of forklift when personnel platform is hoisted.
