A.  On or about March 3, 2006, employer knew or should have known that
employees
involved with critical lift of water tank, using a Terex T750 truck crane,

750168, positioned on south side of the water tank, were exposed to the
hazard of being
struck by the falling water tank due to the total weight of the crane's
load exceeding 70
percent of the rated capacity for the radius and configuration of the
crane.  A feasible and
useful method to correct this hazard, among other methods, is to reduce
the total weight of
the crane's load not to exceed 70 percent of the rated capacity for the
radius and
configuration of the crane.
