SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that five (5) forklift operators, moving pallets of wood in the yard of the establishment, were exposed to the hazard of being thrown from the forklift and crushed due to not wearing a seat belt. A feasible and useful method to correct the hazard, among other methods, is to require employees to wear the seat belt at all times while driving the forklift.

SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that five (5) employees operating the swing cut off saw, no known serial number, in the planer mill, were exposed to the hazard of coming into contact with rotating saw blade due to saw not returning to start position. A feasible and useful method to correct the hazard, among other methods, is to install an automatic return.
