A.  Employer knew or should have known that one operator moving chemicals
in the
shipping and receiving area of establishment was exposed to the hazard of
being thrown from
the forklift and crushed due to not wearing a seat belt.  A feasible means
of abatement would
be to require all forklift operators to wear seat belt while operating
forklift.
