SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards
which may cause death or serious physical harm to his employees and comply

regulation and other occupational safety and health rules and regulations
promulgated under

A.  Employer knew or should have known that the driver not being at the
controls while two
employees were on an elevated platform, in excess of 15 feet high by a JCB
940 model
forklift, north side of building, constituted a hazard of hydraulics
failing and employees not
having a safe way down to ground or of employees falling to the ground.  A
feasible and
useful method, among others, is to have the driver remain at the controls
while employees
are elevated on platform.
