Title 40.1-51.1,(A), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were likely to cause death or serious physical harm to employees in that: (a) Located at The Bassett, Fiberboard Plant, 53 Trent Hill Road, Bassett, Virginia, the employer did not ensure that a work basket was adequately secured to a Cat. Towmotor's lifting carriage or forks. One feasible and acceptable method among others to correct this hazard is to adhere to the ANSI Standard Safety Standard for Low Lift and High Lift Trucks B56.1- 2005-4.17.3(b). Whenever a truck is equipped with a work platform, precautions shall be made for the protection of personnel by an attachment means that is applied to the platform securing it to the lifting carriage or forks.
