Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to: multiple injuries from tip over using an improperly sized
platform (basket) on
a rough terrain forklift:
Elevated work platforms used on rough terrain forklifts did not meet the
design, construction,
and use requirements of the American National Safety Standards/American
Society of Safety
Engineers for the Safety Standards For Rough Terrain Forklift Trucks,
ASME/ANSI B56.6-
2002; Section 8.24.1(b):Floor dimensions shall not have a width greater
than the overall width
of the truck [measured across the load bearing tires plus 10 inches on
either side]. Section
8.24.1(f)(1-2): Information was not prominently indicated on the platform
the maximum working
load indicting personnel and equipment and the weight of the empty
platform.
a) At the site, two employees were working on elevated work platform that
extended a
total of forty-eight (48) inches beyond the wheelbase of the forklift
truck.
b) Information was not prominently indicated on the platform used by two
employees for
the maximum workload including personnel and equiptment and the weight of
the empty
platform.
Among other feasible and acceptable abatement methods to correct this
hazard is to
ensure that all platforms used to elevate personnel with a rough terrain
forklift truck comply with
the requirements of the American Safety Standards for Rough Terrain
Forklift Trucks,
ASME/ANSI B56.6-2002.
