Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to electrical hazards due to the lack of frequent inspections
of overhead hoist(s):
a)  Production floor, by machine #58 -- a Budgit one-ton hoist on a Spanco
1-ton gantry
frame was plugged into an extension cord; the outer insulation had been
pulled from the
connector body, on or about 9/26/06.  Although periodic inspections are
done and
written records are maintained, there are no frequent inspections as
required.
Among other methods, one feasible and accepted method to correct this
hazard is to
perform frequent inspections as per ANSI B30.16-2.2.1.2:
Items such as the following shall be inspected for damage at intervals as
defined
in 16-2.2.1.1 b.1 (daily to monthly intervals) or as specifically
indicated,
including observation during operation for any damage which might appear
between regular inspections.  Deficiencies shall be carefully examined and
a
determination made as to whether they constitute a safety hazard.
a.  All controls and operating mechanisms for improper operation; daily.
b.  All safety devices for malfunction; daily.
c.  Deterioration or leakage in air systems.
d.  Hooks for deformations, chemical damage, or cracks.  Hooks damaged
from chemicals, deformation or cracks opr having more than 15 percent
in excess of normal throat opening or more than 10 degree twist from the
plane of the unbent hook, refer to paragraph 16-2.2.3.3. c.4.  NOTE:
Any hook that is twisted or has throat opening in excess of normal,
indicates abuse or overloading of the unit.  Other load bearing components
of the hoise should be inspected for damage; daily.
e.  Load-carrying ropes or chains.  Visual inspection daily for wear,
twist,
distortion or improper dead-ending to the hoisting drum and other
attachments.  Check chain also for deposits of foreign material which may
be carried into the hoist mechanism.
Abatement certification required within 10 days after abatement date.  The
certification
shall include a statement that abatement is complete, the date and method
of abatement,
and state that employees and their representatives were informed of this
abatement.
