Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that the employer
did not ensure that organic peroxide formulations were properly stored and
handled, and did not
ensure employees were properly trained in the hazards, storage, work
practices, and handling
of organic peroxide formulations, exposing employees to fire and explosion
hazards:
a)  Finishing Room -- The employer did not ensure organic peroxide
formulations were
properly stored, in that two (32) bottles of ILVA TV 84, an organic
peroxide formulation
containing Class III organic peroxides, was observed stored in a flammable
liquids
storage cabinet along with Class 1 flammable liquids including, but not
limited to,
thinners and conversion coatings.  The employer did not ensure storage
areas were
conspicuously identified with the words "Organic Peroxides" and by the
class.  The
employer did not ensure "No Smoking" signs were placed conspicuously at
all entrances
and within storage areas. The employer did not ensure employees were
instructed in: the
proper and safe handling of organic peroxide formulations; personal
protective
equipment; fire suppression equipment; proper and safe disposal of spilled
material; and
proper emergency procedures.  On or about 05/05/2007.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to comply with the requirements detailed in NFPA 432, Code for
the Storage
of Organic Peroxide Formulations.
Pursuant to 1903.19(c) and (d), the employer must provide documentation
that this
violation has been abated within 10 calendar days of the abatement date.
