Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and fire/explosion hazards: Employees working drilling rig #5 at Well# 1-17, SEC 17, TWP 26 in Arlington, Kansas were exposed to struck-by and fire and explosion hazards for the following instances: a)Employees were exposed to struck-by hazards created by the whipping action if high pressure mud hoses in the event of a failure at the discharge from the mud pumps and the connection to the piping at the drill floor. Also the mud hose between these connections was damaged exposing the metal bands internal to the hose. Among other methods, one feasible and acceptable method to correct this hazard is to secure the hose ends using cables or chains properly located and of sufficient strength to resist hose whipping should it become disconnected or broken and/or replace damaged equipment as referenced in paragraph 9.13.4 of API RP 54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operation, 1999 edition. b)Employees were exposed to fire and explosion hazards in that employees were working in the derrick platform prior to installing the auxiliary escape line and the escape equipment was not installed after the line was erected. Among other methods, one feasible and acceptable method to correct this hazard is to install the auxiliary escape line and the escape trolley immediately after the rig is erected and prior to personnel working in the derrick as referenced in paragraph 6.10.1 of API RP 54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operation, 1999 edition. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
