The employer failed to ensure that place of employment was free from
recognized hazards, as
follows:
a.Cascata Golf Course, Cafe Express Turf vehicle, AF 0529-523055, cart
attendants were not provided with proper training in the step-by-step
operating
instructions and safety procedures, required by the manufacturer.  A
beverage
cart attendant was fatally injured on 06-26-06 when she lost control of the
vehicle.  The driver made a sharp turn to the right, struck the curb and
rolled the
beverage cart onto it's side, crushing the attendant underneath.

instructions must be followed in order to avoid the risk of severe
personal injury; Page
18 - Driving Instructions Warnings - drive slowly in turns to prevent

driving the vehicle on slopes exceeding 20incline;
2005 Owner's Manual Supplement For Cafe Express Vehicle, Page 2 - A vehicle
safety video, the Turf Utility Vehicle Owner's Manual and this Owner's
Manual
supplement are provided with the vehicle.  Have all operators view the
safety video and
understand the safety warnings it illustrates, before operating the
vehicle; Page 6 -
Vehicle Operation: the vehicle must not be driven over curbs or on steep
slopes,
fairways, or extremely uneven terrain; Page 8 - an unevenly distributed
load can cause
vehicle instability.
b.Cascata Golf Course, Cafe Express Turf vehicles (AF0529-523056 and
AF0529-
503055), periodic service schedules for Cafe Express vehicles to keep
vehicles
in optimum operating conditions, required by the manufacturer, were not
followed
by the employer.

Tire Specifications: Correct tire pressure is essential.  Tire pressure
must be checked
daily and adjusted on a regular basis; Page 8 - Loading - when stocking
the refreshment
center, make sure that load weights are approximately equal in all lower
compartments
containing ice and beverages. An unevenly distributed load can cause
vehicle instability.
One feasible and acceptable abatement method is to make sure that every
employee assigned to
operate Cafe Express vehicle is trained to recognize hazards associated
with the particular joband to follow step-by-step requirements of the Club
Car - 2005 Turf Vehicles Owner's Manual
and Owner's Manual Supplement for Cafe Express Vehicle.
