The employer failed to ensure that a vehicle-mounted aerial ladder truck
was properly set up
as follows:
(a)At the parking lot of the work site, an employee was performing
electrical
installation work from a Wilkie Mfg. LLC aerial ladder mounted to a Ford
truck.  The employee was sitting on the work platform portion of the
ladder at
a height of approximately thirteen feet above the parking lot below.
(1)The truck was equipped with two outriggers at the rear of the
vehicle, only one of which was deployed (on the building side of
the truck).  The outrigger on the traffic side of the vehicle was
not deployed.
(2)  The wheels of the truck were not chocked to prevent wheel movement.

Order of Operations Procedure, page 3:
-Chock Wheels to prevent wheel movement.
-Extend outriggers to firm support surface, outriggers shall be extended
until truck chassis is partially supported by outriggers.
One feasible means of abatement would be to ensure that the
itemslistedintheOrder of Operations Procedure of the Operating Instructions
Manual are followed by the operators and users of the vehicle-mounted
aerial
ladder.
