Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falls from elevations:
(a) On or about April 21, 2006 an employee was using the Elliott Equipment
Corporation,
extensible boom vehicle mounted aerial lift, Model Number ECH-3-70-H,
Serial Number LC-
1432, which tipped over onto the passenger side of the aerial basket
platform.  The five (5)
outriggers on the vehicle were not fully extended to prevent the aerial
lift from tipping over.
Among other methods, one feasible and acceptable method to correct this
hazard includes but
not limited to the following:
In accordance with American National Standard Institute (ANSI) for Safety
Requirements for
Vehicle-Mounted Elevating and Rotating Aerial Devices, ANSI A92.2 Section
10.8: "..Before
and during each use the operator shall insure that outriggers and
stabilizers are used if the
manufacturer's instructions required their use and the manufacturer's
operating instructions.
The manufacturer's instructions (Elliot Equipment Corporation) requires
the operator to always
be certain all outriggers are fully spread and set firmly; and, before
raising the boom, place the
independently controlled outriggers on solid ground.
No abatement certification or documentation is required for this item.
